Position Summary
The RN/LPN to join the team as a Chronic Disease Nurse. In role, you will provide proactive case management, patient education, and coordinated care to individuals living with chronic conditions such as diabetes, hypertension, COPD, heart disease, chronic kidney disease, and other long-term illnesses. The Chronic Care Management Nurse will report directly to the Home Care Nurse, and under the general supervision of the Health Director.
