Position Summary
The Community Health Nurse will provide community based, culturally relevant programming and services as well as support initiatives and strategies that aim to improve health outcomes associated with maternal, infant, child, and family health. The focus of care is on health promotion, disease and injury prevention, health protection, health surveillance, population health assessment, and emergency preparedness and response. The nurse will provide optimal client services by working with team members in the planning, implementation, delivery, and evaluation of services.
